The short answer

An OTC benefit is an allowance some Medicare Advantage plans include for approved health and wellness items like pain relievers, first-aid supplies and vitamins. Original Medicare does not include it. Amounts, eligible items and how you order vary by plan, so the only way to know what you have is to review your specific plan.

What is a Medicare OTC benefit?

It is an extra benefit offered by some Medicare Advantage (Part C) plans that helps pay for approved over-the-counter health products. It is set by the private insurance company, not by Medicare itself, and it is typically loaded to a card or catalog on a monthly or quarterly schedule.

  • Offered only by some Medicare Advantage plans — not Original Medicare or Medigap
  • Allowance amounts and refresh periods differ by plan and county
  • Unused funds often do not roll over — check your plan's rules

What can an OTC allowance usually be used for?

Most plans limit spending to approved health items. Common categories include cold and allergy medicine, pain relievers, first-aid supplies, dental care products, vitamins and some personal-care items. Each plan publishes its own approved list, and some items may be excluded.

  • Cold, flu and allergy medicine
  • Pain relievers and first-aid supplies
  • Toothbrushes, denture care and other dental items
  • Vitamins and supplements on the plan's approved list
  • Some plans add healthy food or utilities for qualifying members

Who may be eligible?

You generally need to be enrolled in Medicare Parts A and B, live in the plan's service area, and join a Medicare Advantage plan that includes an OTC benefit. Some expanded benefits are limited to members with certain chronic conditions or who qualify for Medicaid.

How do I check whether my plan includes one?

Review your plan's Evidence of Coverage or call the member services number on your card. A licensed agent can also compare plans available in your ZIP code and explain which include an OTC benefit — you can change plans during the Annual Enrollment Period or other qualifying periods.
